Signed enrollment forms must be submitted to the WBOE HR Office to ensure Rx updates will match with your online medical enrollment with the state.  Completed forms can be emailed to eskinner@wboe.net.

Dental Coverage

WBOE dental coverage is offered through Delta Dental's PPO Premier Plan. Standard and Premium plans are available for full-time employees and their eligible dependents, regardless of medical/Rx enrollment.

MEDICAL/RX BENEFITS WAIVER

MEDICAL/RX BENEFITS WAIVER

Please be advised that if you are a full-time WBOE employee and are not enrolled in the district-sponsored medical and prescription coverage, please complete the medical benefits waiver form and return it to the WBOE Human Resources Office by 10/31/2023. The next waiver reimbursement check will be issued in January 2024 and takes into consideration the months of waived coverage from July through December 2023. Having this documentation on file ensures proper reimbursement amounts are being issued. If you have eligible dependents that you are also waiving coverage for, please include the requested information on this form. Eligible submissions received after 10/31/2023 will be paid out in June 2024.

As a reminder, employees must waive medical and Rx coverage for one year before being eligible for the waiver reimbursement program.

Employees covered by NJ-sponsored health plans are not eligible for monetary reimbursement.

Updated waiver forms will be due annually.
