***We are pleased to announce that students will have a chance to join a team and participate in sports at the middle school once again.

There are certain requirements that must be met in order for students to participate with the sports program. Please download the attached form, sign after reading it and have student return to school.

**Physicals are mandatory prior to participation in any sport. Physical packets can be obtained from the Main office, School nurse or see Ms. Baczynski.
