We are pleased to share an important community resource in partnership with Southern Jersey Family Medical Centers (SJFMC), focused on supporting families impacted by sickle cell disease.
Sickle cell disease is a serious condition that can begin affecting children as early as 6–7 months of age and requires consistent, coordinated care to manage symptoms and prevent complications. It impacts more than 100,000 individuals in the United States, and awareness and access to care are critical for improving health outcomes.
SJFMC is offering a “Live Well with Sickle Cell” Pilot Program, designed to provide comprehensive, coordinated care and support for individuals and families. Program services include:
A Primary Care Medical Home for patients with sickle cell disease
Ongoing medication evaluations and health monitoring
Care coordination and access to specialists
Wraparound services, including transportation and interpretation
Reproductive health services
Nutrition education and support
